Advanced Implant Reconstruction in Vero Beach, FL

Some implant cases are straightforward: one missing tooth, healthy bone, a clear path to placement. Others are not. Patients arrive at our practice every week with cases that other offices have either declined or were unable to resolve successfully. These are the cases Dr. J. Andrew Colgan has spent his career mastering.

Advanced implant reconstruction is the work of restoring teeth in patients who have severe bone loss, failed prior implants, full or near-full edentulism, complex medical histories, or aesthetic and functional demands that exceed routine implant treatment.

Cases We Take On

Patients are referred to us (or come to us directly) for advanced reconstruction when their situation includes one or more of the following:

  • Severe bone loss from long-term tooth absence, periodontal disease, or trauma
  • Failed prior implants that need to be removed and replaced
  • Full arch restoration when most or all teeth are missing or failing (see Full Arch Restoration)
  • Patients told elsewhere that implants are not possible without major hospital-based grafting
  • Medical complexity including a history of head and neck radiation, bisphosphonate therapy, immunosuppression, or systemic conditions that affect healing
  • Cleft palate or other congenital conditions affecting the jaws
  • Severe atrophy of the upper or lower jaw
  • Aesthetic zone challenges where bone, gum tissue, and tooth proportions all need to be addressed together

How We Approach Complex Cases

Advanced cases require advanced planning. Our process typically includes:

Comprehensive consultation. A complete review of your medical, dental, and surgical history, with 3D cone beam imaging to map your existing anatomy and any pathology.

Multidisciplinary planning. Many advanced cases involve coordination with restorative dentists, ENTs, and primary care providers. Dr. Colgan has worked with referring providers in the Treasure Coast area for over 20 years, and these relationships translate into smooth, well-coordinated care.

Custom regenerative strategy. Some cases require bone grafting, sinus lifts, ridge augmentation, or a combination, often enhanced with PRP or BP. Other cases can be approached with techniques that avoid major grafting, such as zygomatic implants or tilted implant strategies, depending on anatomy.

Staged or immediate restoration. Some patients are best served by a staged plan over several months. Others, especially those preparing for full arch restoration, are candidates for fixed teeth in a day using Dr. Colgan’s proprietary Hybrid Express Platinum protocol.

Why Dr. Colgan

Dr. J. Andrew Colgan brings credentials that few practices can match for advanced cases:

  • Diplomate of the American Board of Oral and Maxillofacial Surgery
  • Fellow of the American Association of Oral and Maxillofacial Surgeons
  • Fellow of the American Society of Implant and Reconstructive Dentistry
  • Over 20 years of full-time oral and maxillofacial surgery practice
  • National lecturer on advanced implant and reconstructive techniques
  • Developer of the proprietary Hybrid Express Platinum Full Arch protocol

For complex cases, experience and judgment are not optional. They are the difference between predictable success and the kind of failures that drove the patient to seek a second opinion in the first place.

What to Expect at Your Consultation

We block extra time for advanced consultations so we can do a thorough exam, capture imaging, and give you a clear picture of your options at the same visit. You leave with:

  • An honest assessment of your case
  • One or more treatment options with the rationale for each
  • A written cost estimate
  • A timeline you can plan around
  • Time to ask questions

Some patients arrive expecting to be told that nothing can be done. Most leave with a plan.

Frequently Asked Questions

I was told I do not have enough bone for implants. Can you help?

Often, yes. Advances in grafting, biologics, and implant technique have made many cases possible that would have been impossible a decade ago. We will look at your imaging, review what you have been told, and give you an honest answer.

What if my prior implants failed?

Failed implants can usually be removed, the site can be regenerated, and new implants placed. Understanding why the prior implants failed is the first step. Dr. Colgan reviews the history carefully before recommending a path forward.

Are advanced cases more expensive?

Generally yes, because they involve more visits, additional grafting or biologics, and more complex surgical planning. We provide a written estimate at consultation and offer financing through PatientFi and CareCredit.

Will my treatment be done in the hospital?

Most advanced implant cases are completed in our office under IV sedation. Cases requiring extensive jaw reconstruction or general anesthesia in a hospital setting are uncommon but can be coordinated when needed.

How long does treatment take?

Timelines vary widely. Staged grafting and implant cases typically take six to twelve months. Full arch cases using the Hybrid Express Platinum protocol can deliver fixed teeth on the day of surgery, with the final restoration completed over several months.

Can I have full arch implants if I have severe bone loss?

In most cases, yes. Strategies include grafting prior to implant placement, using longer or angled implants placed in the available bone, or, in select cases, zygomatic implants that anchor in the cheekbone. Dr. Colgan will discuss which option fits your anatomy.

Are dental implants safe for patients with medical conditions?

Many patients with chronic conditions (diabetes, heart disease, autoimmune conditions) can safely have implants. Some conditions require special precautions, and a few are contraindications. Dr. Colgan reviews your medical history with you in detail.

Do you take patients from out of the area?

Yes. We see patients from across the Treasure Coast and beyond. Out-of-area patients can coordinate consultation and treatment around their schedule. See Areas We Serve for more.
