Bone Morphogenic Protein (BMP) for Advanced Bone Regeneration

Bone morphogenic protein (BMP) is a biologic agent that directly signals the body’s own cells to form new bone. In oral and maxillofacial surgery, BMP is used selectively when the bone defect is large, when traditional grafting is unlikely to produce the volume needed, or when harvesting a patient’s own bone (autograft) is undesirable.

Dr. J. Andrew Colgan uses BMP in carefully selected cases at Vero Beach Surgical Arts, often in combination with bone grafting materials and platelet-rich plasma (PRP).

What BMP Does

Bone morphogenic proteins are naturally occurring signaling molecules in the body. They are part of the system that tells stem cells and other healing cells to differentiate into bone-forming cells.

The form of BMP used in surgical practice (recombinant human BMP-2, or rhBMP-2) is a purified version of this signaling molecule, delivered to the surgical site on a collagen sponge carrier. Once placed, BMP recruits the patient’s own cells to form new bone at the site.

The key advantage: BMP can stimulate bone formation in defects that are larger than traditional grafting alone can reliably reconstruct.

When BMP Is Considered

BMP is not used for every case. It is reserved for situations where its specific benefits are meaningful:

  • Large bone defects following extraction, trauma, or cyst removal
  • Sinus lift procedures where significant vertical bone gain is needed (see Sinus Lift)
  • Ridge augmentation where the residual bone is severely deficient (see Ridge Augmentation)
  • Failed prior grafts that need a more aggressive regenerative approach
  • Patients who decline autograft (their own bone harvested from another site)
  • Complex full arch implant cases where bone volume is the limiting factor

For most routine grafting, traditional bone graft materials (with or without PRP) produce excellent results. BMP is added when the clinical situation justifies it.

Safety and Considerations

BMP has been used in oral and maxillofacial surgery for many years and has well-documented benefits in appropriate cases. As with any biologic, certain considerations apply:

  • Mild swelling at the site is common during the early healing phase
  • BMP is not used in pregnant patients or in patients with active cancer in the area
  • Some patients are not appropriate candidates based on medical history

Dr. Colgan reviews your medical history thoroughly and discusses the risks and benefits in detail before recommending BMP.

What to Expect

BMP is delivered during the same surgical visit as grafting or implant surgery. There is no separate procedure to receive BMP, and there is no recovery beyond the recovery for the underlying surgery.

In the months following placement, the BMP signals new bone formation in the grafted area. Imaging at follow-up visits confirms the result before implant placement (in staged cases) or after integration (in immediate cases).

Frequently Asked Questions

Is BMP safe?

Yes, when used in appropriate cases and at appropriate doses by an experienced surgeon. BMP has a well-established safety profile in oral and maxillofacial applications.

Will BMP replace traditional grafting?

Not in most cases. Traditional bone graft materials (allograft, xenograft, synthetic) work very well for routine cases. BMP is added when the case is large or complex enough to benefit from a stronger regenerative signal.

How is BMP different from PRP?

PRP concentrates your own platelets and growth factors from your blood. BMP is a purified, recombinant version of a single specific signaling molecule. They have different mechanisms and are sometimes used together.

Does BMP cause unwanted bone growth?

Properly contained at the surgical site, BMP stimulates bone where it is delivered. Concerns about uncontrolled growth have been associated mostly with off-label use at high doses in other anatomical regions. In oral surgery, where BMP is used at well-defined doses and within contained surgical sites, this is not a concern.

Will my insurance cover BMP?

Coverage varies. BMP is sometimes covered as part of a medically necessary grafting procedure and sometimes not. Our team verifies coverage and provides a written estimate.

Will BMP help my graft work better?

In appropriately selected cases, yes. BMP can improve bone formation in challenging defects. It is not a guarantee, but it is a powerful tool in complex situations.

Is BMP from a human or animal source?

The BMP used in surgical practice (rhBMP-2) is recombinant, meaning it is produced in a laboratory rather than harvested from a human or animal source.

How is BMP delivered?

BMP is applied on a collagen sponge carrier at the surgical site at the time of grafting or implant placement. It does not require a separate procedure.
